Quick Answer: Passport Annexure F is the specimen declaration you sign to explain how and when your passport was lost or damaged. Passport Seva needs it for every re-issue of a lost, stolen or damaged passport. It's a self-declaration on plain paper, and no stamp paper or notary has been needed since December 2016.
The Annexure F passport form has six short questions about the loss, any FIR, travel on the old passport, time abroad, past objections and deportation. Download it from the Passport Seva Annexures page. An older file that also says "Annexure F" is a Tatkaal verification certificate, not this form.

What is Annexure F for a passport?
Annexure F is the declaration Passport Seva asks for when you want a new passport in place of a lost or damaged one. You write out what happened, affirm it's true and sign it.
It ends with a promise to take "utmost care" of the new passport. You also accept that the government may act under the Passports Act, 1967 if the loss happens again. Passport Seva's document list calls it an affidavit "stating how and where the Passport got lost/damaged". So a checklist asking for a "lost passport affidavit" means this form.
When do you need Annexure F?
You need Annexure F whenever you apply for re-issue because your passport was lost, stolen or damaged. That includes a damaged passport you still hold.

This follows the re-issue document table in Passport Seva's instruction booklet, where Annexure F is document 49. If you need the passport quickly, note that lost, stolen and damaged-beyond-recognition passports can't use Tatkaal. A readable damaged passport can, though the Passport Office has the final say.
How do you fill in Annexure F?
Write your name, relation line (S/o, D/o or W/o, meaning son, daughter or wife of, then that person's name) and present address, answer all six questions in plain sentences, then date and sign it.

Before you sign, check three things:
- Every question has an answer. Write "No" or "None" instead of leaving a blank.
- If you have a police report, the date and police station match it exactly.
- The closing affirmation is unchanged. Don't edit the printed wording.
If you can't give the stamp page numbers for question 4 because the passport is gone, ask your Passport Office what it will accept. Never copy a "filled sample" from the internet; your answers must describe your own case.
Does Annexure F have to be on stamp paper?
No. The Ministry of External Affairs announced in December 2016 that every passport annexe would be a self-declaration on plain paper, with no attestation before a notary or magistrate. Read the MEA announcement of the new passport rules on Passport Seva. Print the form on ordinary paper and sign it yourself.
Where do you download the current Annexure F?
Download it from the Passport Seva Annexures and Affidavits page, under "Specimen Affidavit for a Passport in lieu of lost/damaged Passport". The direct English file is the current Annexure F PDF, the same one Passport Seva's document advisor uses.
Avoid a second file on the same site named AnnexureF_V1.0.pdf. It's an older file, a "Specimen Verification Certificate (for Passport under Tatkaal only)". The Passport (Amendment) Rules of 23 December 2016 replaced the whole annexure set, so that old lettering no longer applies. If your download asks an officer to certify your character, it's the wrong file.

What else goes with Annexure F in a lost passport application?
Annexure F doesn't replace the police report. A lost or stolen passport also needs a police report in original, proof of address and proof of date of birth. A readable damaged passport goes in with the damaged booklet itself and proof of date of birth (plus proof of address if it has changed).
