What is passport Annexure E?
Passport Annexure E is a signed declaration in which an applicant confirms ten statements about themselves. Passport Seva's instruction booklet lists it as document 48, a "Standard Affidavit as per Annexure E".
You'll find the current Annexure E PDF on passportindia.gov.in. Its text matches the Annexure E that the MEA's Passport (Amendment) Rules of 23 December 2016 added to the Passport Rules, 1980.
Before 2016, "Annexure E" was the affidavit for getting a passport in a changed name. The Permanent Mission of India, an MEA office, confirms that affidavit was discontinued. For a name change you now give two newspaper clippings or a Gazette notification, plus documents in the new name.
As of 22 September 2026, Passport Seva's Annexures/Affidavits page lists Annexures A, C, D, F, G, H, I and J and Appendix 12, but not Annexure E. The Annexure E PDF still loads from its own link, and the police verification mode page still names it.
When does Passport Seva ask for Annexure E?
Passport Seva asks for Annexure E when a first-time applicant wants a normal passport processed on post-police verification. You submit it with Aadhaar, voter ID and PAN, and your Aadhaar number must pass online validation.
Passport Seva's police verification mode details set out the three modes:

How do you fill in Annexure E?
To fill in passport Annexure E, write your name, relation line, address and date of birth at the top. Then fill points 1 and 2, pick the right options in points 3 and 7, and sign at the bottom. Here's a filled-sample guide to each field:

What mistakes should you avoid on Annexure E?
Check these before you sign passport Annexure E:
- Your name, parents' names and date of birth match the application form exactly.
- Every blank is filled, and the citizenship words that don't apply are struck out.
- Each statement is true for you, because points 4 to 6 cover recent serious convictions, pending criminal cases and court warrants.
Does Annexure E need stamp paper or a notary?
No. The MEA's announcement of the new passport rules says every annexe is a self-declaration on plain paper. No notary or magistrate attestation is needed. Print Annexure E on ordinary A4 paper and sign it yourself.
